Hello,
1. You need to have First Name (Stored in DB) and Last Name (Stored in DB) field in your form, not the full name field.
2. Notification email looks fine on mobile devices. Could you please share screenshot?
3. Page jumps to element which has validation issue. If you do not like this, under Form Type, please disable Scroll to element on validation error
