Please this is very important. I want to make a table. I can not make it in any other form and my forms have many tables

u told me once about <label class=”ipt_uif_question_title ipt_uif_label” for=”ipt_fsqm_form_13_freetype_0″>Feedback Matrix Title</label> but i can not make multi seleect inside it or any other radio button, it is only fill in the text boxes?
2)ADD ANOTHER FILE for the attachments, can i make it? how?
