Form stopped allowing submissions once conditional logic was applied

Hi,

I’ve added conditional logic to my form but now when I press the submit button it doesn’t work at all. I get the “Processing your request” text and then it just goes blank.

What could be causing this? And how do I fix it?

Edit: Error message:

locationpicker.jquery.js:180 Uncaught TypeError: Cannot read property ‘Autocomplete’ of undefined this is error autocomplete
