1. Found a solution. Just used the single slider, appointed the values I needed and made each label a separate question/single slider and got the numeral ratings as needed when using the Easy SQL for FSQM Pro (just connected to the database and then downloaded as .xls). Although it would be great if we could point numeral values to checkbox options and what-not, this seemed to satisfy our primary need.
2. Yes, we removed the matrix element since it wasn’t what we were looking for and the single slider looked fine, even when scaling.
3. Exporter didn’t do the trick for us unfortunately, the .csv got all messed up for some reason. Purchased the SQL add-on also and that gave us the .xls files we needed.
