Conditional Fields Not Refreshing

Hi FSQM,

First of all, excellent form plugin.

We have a problem with conditional fields though. Specifically, fields do not hide themselves once the conditions are no longer met.

Example:

If user selects A -> show X
If user selects B -> show Y

Expected Outcome:

If user selects A -> show X
If user deselects A and clicks B -> Hide X, Show Y

Actual Outcome:

Both X and Y show, X does not hide itself.
